As I manage all appointments personally without a practice administrator, I use my online booking system to keep everything organised and running smoothly - this is why I need to ask all clients to complete the full registration process below when booking.
1. Steps to complete your online registration
-
You need to complete the full online registration for your appointment request to be honoured.
-
After starting, please wait for a confirmation email with your code before continuing.
-
Complete all fields where possible, including: Credit card details (held as a booking guarantee), Medicare card details and expiry date, Full name, date of birth, and address, Contact details (including your GP and an emergency contact)
2. Review and sign your documents
-
You will receive a separate email via Adobe Sign with the Psychology Contract
Please read these carefully - particularly the details regarding cancellations and policies - and complete and return them promptly. The contract will be provided.
3. Provide required supporting information
-
Before your appointment can be confirmed, we require your completed Online Registration and signed Client Contract forms. If you would like to access Medicare rebates, you will also need to provide a valid GP Referral and Mental Health Care Plan prior to your appointment.
Important:
We are unable to honour or confirm any appointment requests until all of the above steps have been completed and received.
Cancellations: Life happens and we understand that you may need to cancel your appointment. Appointments require a minimum of 48 hours’ notice for cancellation or rescheduling; late cancellations and non-attendance (no-shows) may incur the full session fee, however consideration may be given in cases of illness, particularly if an alternative appointment can be offered within the same week.
Workers Compensation Claims: Even when your sessions are part of a workers’ compensation claim, you’ll still be asked to provide your credit card details when booking. This is because coverage is subject to approval and review processes, and may not be confirmed at the time of your appointment. Providing card details simply allows the appointment to be secured, and ensures there is a backup method in place should sessions not be approved or if there are any delays in payment. Kindly note that any active claim will usually mean that expenses will be refunded by your insurer.
